The Caritas Christi Aged Care Unit provides assessment and treatment of conditions associated with ageing. The Unit aims to help achieve maximum independence and quality of life. It also provides a short-term home-based service which assits people to manage better in their own homes. |

| The Aged Care Unit is located at Caritas Christi Fitzroy. Our Aged Care Unit, and associated home-based service, is located on level 3, Bolte Wing, St. Vincent's Hospital Melbourne. | |||
| The Caritas Christi Aged Care Unit provides assessment and treatment of conditions associated with ageing. The unit aims to help clients achieve maximum independence and quality of life. It also provides a short-term home-based service, which assists clients in managing better in their own home. | |||
| After admission, members of the Aged Care Team assess clients'
needs and establish goals. The team may include medical and nursing staff, social workers,
occupational therapists and physiotherapists. Staff from other disciplines may also be
involved. Regular team meetings are held to monitor clients' progress. Meetings are also arranged with family and carers to plan for the time following discharge. |
